Through a series of guided personal-data-driven coaching sessions we're able to sustainably improve healthcare outcomes for patients, reduce healthcare costs for funders and improve medication adherence for pharma companies. Our mobile-first software platform provides for team-based care coordination, auditable voice call recording and systematic treatment protocol adherence. Our care providers are empowered to work from any location to serve all segments of the population, connecting those who need care with those best suited to provide it. We maintain clinical quality assurance and provide analytics to funders. Currently serving almost 2,000 patients for one of the world's largest pharma companies in diabetes management, we're expanding into renal care and seek funding to expand the product and our provider network.
